Minnesota Specialty Health System (MSHS) in Wadena is seeking a talented, mission driven, person-centered Discharge Social Worker to provide discharge planning services to MSHS Wadena staff. MSHS Wadena provides rehabilitation and supportive residential services for individuals with serious and persistent mental illness. The Discharge Social Worker will provide integrated social work services at Minnesota Specialty Health System. Under the direction of the Treatment Director this position provides referral recommendations and coordinates discharge supports. This position also provides back up to the intake coordinator by completing client referral interviews, completing the preadmission assessment and processing admissions.
The incumbent of this position must be flexible in terms of work hours as well as the ability to travel.
Primary responsibilities include:
Coordinating and completing the discharge planning process and providing back up support to the intake coordinator.
Working with the interdisciplinary treatment team to establish goals for clients and recommend methods by which these will be attained, including target dates to facilitate transitioning back into the community.
Attending discharge planning meetings with the treatment team to identify treatment barriers to discharge, prepare aftercare plans, assure timely discharge, and complete transfer summaries when applicable.
To qualify, applicants must meet the requirements for a Mental Health Practitioner as defined in Sec. 245I.04 MN Statutes, subdivision 4 and must meet the following criteria:
• Bachelor’s degree in psychology, social work, behavior analysis, or related field; and
• Licensure/Certification in the State of Minnesota as a Licensed Social Worker.
• Two years of post-bachelor’s degree experience providing behavioral health clinical services including at least 3 of the 5 activities listed below:?
o Delivering psychoeducation and/or behavioral interventions?
o Providing case coordination among multidisciplinary team members?
o Conducting comprehensive assessments?
o Leading development of treatment/support plans as required by program licensure?
o Planning discharge/transition and/or aftercare?
OR
• Master’s degree in psychology, social work, behavior analysis, or related field;
• Licensure/Certification in the State of Minnesota as a Licensed Social Worker
• One year of post master’s degree experience providing behavioral health clinical services including at least 3 of the 5 activities listed below:?
o Delivering psychoeducation and/or behavioral interventions?
o Providing case coordination among multidisciplinary team members?
o Conducting comprehensive assessments?
o Leading development of treatment/support plans as required by program licensure?
o Planning discharge/transition and/or aftercare?
OR
• Master’s degree in psychology, social work, behavior analysis, or related field; and
• Independent Licensure/Certification in the State of Minnesota as an Licensed Independent Clinical Social Worker (LICSW)
• Knowledge of or experience working in R245I
• Knowledge of the civil commitment process to understand the effects of discharge planning for clients and their care
