As a Behavior Modification Assistant (BMA) you will be responsible for supporting individuals who are in our short- and long-term housing program. These individuals may have intellectual disabilities, mental illness, brain injury, and other emotional, behavioral, medical complexities. In this position you will ensure the people we serve achieve their highest quality of life and personal growth! You will do this by: • Assisting in assessing individual's needs by performing direct care duties • Developing and implementing residential and habilitation support plans with goals and methods. • Evaluating and documenting out-comes, maintaining or modifying the plans and performing related work as required. • Respond therapeutically using de-escalation strategies and positive behavior supports to individuals with challenging behaviors. Site Description 89th Crescent in Brooklyn Park. ? Urban setting yet quiet neighborhood ? EASE Level 3 ? 24-hour awake staffing, with varying individual supervision needs ? Non-identifying description of the individuals, include the following: o Age range of current residents is 30’s-70’s o The physical and emotional needs of the individuals in the home include having a diagnosis of: Pervasive Developmental Disorder, PCOS, Morbid Obesity, Hyperglycemia, Hyperlipemia, Arthritis, Attention Deficit Hyperactivity Disorder, combined Generalized Anxiety Disorder secondary Borderline to mild Intellectual Functioning, Borderline Personality Disorder, Mild ID, OCD, Depression, Hypertension, Diabetes, Profound ID, Conduct Disorder, Autistic Disorder, Bipolar Disorder, Seizure Disorder(Grand Mal or Generalized Tonic-Clonic), Gait instability, Weakness generalized, Hx Aspiration pneumonia, Acute hypoxic respiratory failure o Two individuals are ambulatory, One is Semi- ambulatory and the other individual uses a wheelchair. *Site description narratives are subject to change at any time as individuals residing in the home or program or their level of care required may change.

PLEASE NOTE: Any time an employee bids, voluntarily transfers, or voluntarily demotes to fill a vacancy in an existing MSOCS, any previously approved vacation may be rescinded if any employee already working in the MSOCS has approved leave for any of the days.
