This position is responsible for leading the Behavioral Health Intensive Residential Transitions policy area. The focus of this position is working to resolve problematic transitions and discharges from intensive residential and healthcare levels of care. This includes identifying and addressing challenges and gaps in the mental health continuum of care for adults, children, and adolescents, and concentrating upon those areas that negatively impact an individual’s ability to return to their community. This position will lead efforts to examine barriers to successful discharge and work with providers, counties, and tribes to support resource development and creative problem solving necessary to ensure a smooth discharge or transition back to the community for individuals boarding in hospitals or intensive residential facilities. The incumbent must be able to work quickly and efficiently with multiple highly visible and challenging situations at the same time. The incumbent must develop key, strategic partnerships with hospitals and residential treatment facilities, counties, tribes, community-based providers, managed care organizations, Behavioral Health Administration (BHA), Department of Children, Youth and Families, Disability Services Division and other stakeholders and family partners. This position shall maintain an extensive working knowledge of community resources; and will facilitate and coordinate continuous communication with stakeholders that result in an increased collaboration and ability to quickly and creatively resolve impediments to discharge and transition to lower levels of care. This position will not only provide consultation and recommendations for discharge resources but will directly connect with those resources to ensure availability, advocate for eligibility, follow up on referrals, and support individuals and families throughout transition to a lower level of care. The incumbent will gather data, complete project and outcome reports and evaluations of processes and procedures. And will lead department efforts to analyze the data to develop new transition strategies, recommend new service development, and expand service array. Some Responsibilities Include, but not limited to: -Act on behalf of the Behavioral Health Administration as policy lead to create, implement, monitor, and oversee the department’s efforts to transition individuals from intensive residential and healthcare settings to other lower level of care options which best meet the needs of the individuals involved. As part of the Intensive Residential Services team, this position is responsible for leading and coordinating efforts between BHA, Department Human Services licensing, Aging and Disabilities Services Administration, DCYF, hospitals, counties, tribes, individuals and families and other stakeholders. -This position exists to provide leadership and oversight of the creation of a state-wide mental health community capacity building initiative. This position must work collaboratively with the Residential Intensive Services team, BHA, and other stakeholders to develop, expand, and implement behavioral health community-based treatment and services, including early intervention and preventative resources.
